Diet for Smokers and Ex-Smokers

By Linda Smyth As a dietitian, the three most common questions which smokers ask me about diet and smoking, are: (1) Can a diet compensate for health damage caused by smoking? (2) What should I eat if I smoke? (3) If I quit, what type of diet is best to prevent weight gain? (1) CAN A DIET COMPENSATE FOR SMOKING? No diet or eating-plan, no matter how nutritious, can neutralise the health damage caused by the 850+ chemicals in tobacco smoke - many of which are carcinogenic. On average, if you smoke 20 cigarettes a day, you double your risk of a heart-attack and are five times more likely to suffer a stroke than a non-smoker. At 40 cigarettes a day, you are five times more likely to suffer from sudden cardiac death. Smoking is also the leading cause of lung cancer. Thus before we even start to examine an appropriate type of diet for smokers, my overriding advice is: quit smoking today! (2) WHAT SHOULD I EAT IF I SMOKE? For anyone who smokes, daily diet nutrition is critical. The damage doneto the body’s cardiovascular and respiratory functions requires a constant need for extra nutrients. Even if you smoke 5 cigarettes a day, you have increased nutritional needs due to your increased risk of hypertension, atherosclerosis, emphysema and numerous cancers. As stated, a healthy diet will not prevent these health conditions, but it may delay their development. INCREASED NEED FOR ANTIOXIDANTS Tobacco smoke leads to increased levels of free radicals - cancer-causing agents - in the body and a corresponding need for protective antioxidants that can neutralise them. The main antioxidant vitamins are vitamin C and vitamin E (which works best in combination with the mineral selenium). Phytochemicals such as bioflavonoids and carotenoids (eg. beta-carotene) are also rich in antioxidants. HOW TO INCREASE YOUR ANTIOXIDANT INTAKE Use the following suggestions are a guide to minimum dietary requirements. - Eat 3-5 daily servings of deep green, dark red, orange of yellow vegetables.- Eat 3-5 daily servings of red, yellow, orange or green fruits.- Switch from coffee to tea, ideally green tea.- Each day, take 2 tsp of wheatgerm oil (rich in vitamin E) and 6 Brazil nuts (selenium). [Note: a serving is approx 1 medium fruit, or 1/2 cup chopped] SPECIAL NEED FOR INCREASED VITAMIN C One cigarette is estimated to rob the body of 25mg of vitamin C. Thus all smokers have a greatly increased need for this antioxidant-rich vitamin simply to maintain minimum levels. In practice, this need can only be met by taking supplements. As a general guide, I suggest you take 1 gram of vitamin C supplements per day. Choose a “timed-release” brand that includes a minimum of 100mg bioflavonoids. BEST DIETARY SOURCES OF VITAMIN C Fruits, such as: blackcurrants, papaya, guava, cantaloupe, elderberries, kiwi fruit, mango, oranges, strawberries. Fruit Juices, such as: cranberry, grapefruit, lemon, orange. Vegetables, such as: red peppers, green peppers, broccoli, Brussels sprouts, cauliflower, kale, tomatoes. PROTECTIVE CAROTENOIDS Carotenoids are pigments found in plants. All carotenoids are antioxidants, the most common example being beta-carotene. It is found in green plants (eg. spinach), as well as orange and yellow plants, such as carrots, sweet potato and melons. Clinical trials indicate that when consumed in foods - not supplements - betacarotene reduces certain precancerous symptoms. [Warning: for reasons which are still unclear, beta-carotene supplements actually increase the risk of disease (eg. lung cancer). Thus your intake of beta-carotene should come exclusively from food.] HOW TO INCREASE YOUR INTAKE OF CAROTENOIDS Use the following suggestions are a guide to minimum dietary requirements. Eat 4 daily servings of deep green, yellow or red vegetables, including: carrots, sweetcorn, pumpkin, spinach, sweet potato. Eat 4oz tomatoes daily, either in sauce or chopped. Eat 3 daily servings of colored fruit, including: melon, oranges, strawberries, mango, cherries. PROTECTIVE BRASSICAS According to research evidence, cigarette smokers who eat more brassicas have less incidence of cancers (eg. breast, colorectal, lung, pancreatic, prostate and stomach). As all these cancers are initiated by free radicals, it follows that brassicas may help to prevent other problems initiated by free radical damage and accelerated by smoking, such as: cataracts, emphysema, asthma and age spots. Brassicas include: broccoli, Brussels sprouts, cabbage (all types), cauliflower, cress, horseradish, kale, kohlrabi, mustard, radish, swede, turnip, watercress. GARLIC AND ONIONS Garlic is a good source of unique antioxidants and contains anti-bacterial and anti-viral properties. Its anti-tumor properties are well documented. Onions, a member of the same vegetable family, have similar properties to garlic. MORE DIETARY TIPS FOR SMOKERS Reduce the total fat in your diet. At the same time, minimize your intake of saturated fat and trans-fatty acids. Eat regular servings of omega-3-rich oily fish (eg. salmon, mackerel, sardines). Eat healthy carbohydrates. Avoid refined white flour carbs, choose only whole grains such as, oats, brown rice, wholewheat pasta. In addition, choose foods rich in soluble fiber (eg. apples, oat bran). Eat healthy low-fat protein such as fish, lean chicken/turkey, or egg-whites. Include small amounts of lean red meat in your diet, along with regular servings of soy foods (eg. soybeans) and other vegetable protein. Reduce sodium in your daily diet. Check food labels and choose low-sodium or sodium-free foods. Also avoid adding salt when cooking or eating. TAKE REGULAR CARDIO-AEROBIC EXERCISE No cigarette smokers diet-plan is complete without regular physical exercise. Working within your fitness capacity, gradually increase the intensity and duration of your workouts to about 30-45 minutes a day, on most days. For best effects on lung capacity and cardiovascular function, choose aerobic exercise such as: brisk walking, jogging, jumping rope, swimming and most sports. (3) CAN YOU PREVENT WEIGHT GAIN AFTER YOU QUIT? Yes, if you take proper physical exercise and eat a healthy calorie-controlled diet, you are unlikely to gain weight. However, in my experience, some weight gain seems to be inevitable. WHAT IS THE AVERAGE WEIGHT GAIN? Most smokers gain weight immediately after they quit. Current evidence suggests that the average weight gain for both men and women who quit smoking is about 6-8 pounds. The more you smoke, the higher the risk of weight gain when you quit. This weight gain is due to the decrease in metabolic rate and an increase in appetite experienced when you quit smoking. This weight increase is perfectly normal, and need only be a short term event. SO WHAT SHOULD I EAT? There is no single diet which will prevent weight gain once you quit smoking. Your best option is to focus on healthy eating combined with regular vigorous exercise (within your fitness capacity) and let Nature do the rest. As a general guide, follow these suggestions. First, cut down on caffeine. Nicotine withdrawal makes us jittery and nervous. So it’s important to avoid coffee and caffeine-rich soft drinks which may increase this nervous tension. Second, increase your intake of fresh fruit and vegetables. Studies show that an increased intake of fruit and vegetables can help to minimize weight gain after you quit smoking. Eat them for snacks, add them to meals, eat them as starters and/or desserts. Eat them on car journeys instead of sweets or candy. Third, eat little and often. Avoid the temptation to linger over your meals. Get into the habit of eating smaller meals at more regular intervals. Aim to eat something, no matter how small, every 2-3 hours. This helps to maintain a regular rate of calorie-burning. Fourth, take steps to learn more about nutrition and choose nutrient-dense foods whenever possible. (See above for information about antioxidants, and healthy fats, carbs and protein.) WHAT ELSE CAN I DO TO PREVENT WEIGHT GAIN? Make physical exercise a top priority in your daily schedule. Ideally join a gym or fitness center and get into shape. Research evidence demonstrates a clear link between exercise and weight control after you quit smoking. Choose both cardio-aerobic and strength-training exercises, as both play an important role in raising metabolic rate. Also, make sure you get enough sleep. Research evidence shows that lack of sleep may lead to weight gain, as well as an increased craving for cigarettes and food. Wooden spoon puppets are easy to make, buy some cheap wooden spoons from the pound shop or supermarket, and draw on a happy face on one side and a sad face on the other with felt pens. Use one of the bottles from the bowling to throw rings over. These can be cut from the side of large cardboard boxes if required. 18. If you have small children try and get a very large box from a supermarket and decorate it as a car to sit in. Paper plates can be used as wheels and steering wheel, flattened small foil pie cases as hub caps and lights, any paint for the bodywork and use a parker pen to draw the doors in. 19. Try blow football, any light ball (practice golf balls are cheap) and a straw will do the job. 20. Dress up, any old clothes, hats and shoes will do, but scarves and lengths of fabric will give more options. One direct marketing expert goes so far as to credit the headline with 100% of the success or failure of any ad or sales letter, because if the headline fails to pull the reader into the main body of the ad or sales copy, then there is no chance of success! And they are both right! Instead, you quickly scan the headlines, looking for something that interests you, either because of curiosity or because there might be something in it for you. You skip over all the rest! The time it takes a reader to scan most headlines and decide whether or not to stop and read your copy or move on is less than 3 seconds! As a result, many professional copywriters spend as much time creating their headlines as they do for the rest of their copy. Know Your Medical Privacy Rights - Be Smart, Be Safe

You keep insurance on your house, get the oil changed regularly on your car, and most important, when you or your family are sick, you get the best healthcare you can. What about your healthcare information? Are you doing everything you can to make sure that your private health information is protected? Do you know what your healthcare privacy rights are? Congress passed the Health Insurance Portability & Accountability Act (HIPAA) in 1996. The HIPAA Privacy Rules, which were created by the Department of Health & Human Services, became effective in April 2003. You now have the rights you need in order to make sure that your healthcare information is safe. If you have any concerns that it isnt, then you can complain to your provider or complain directly to Health & Human Services. Rest assured, should you complain to Health & Human Services, your complaint will be investigated. Since April 2003, more than 15,000 complaints have been received and addressed. So, what are your healthcare privacy rights? First, all healthcare providers such as doctors, dentists, optometrists, etc., are required to give you a copy of their Notice of Privacy Practices. Has yours? Each time you visit a new provider for the first time, their office should give you a copy of their Notice of Privacy Practices. If they dont — insist that they do. The copy of the Notice must be one that you can take with you, or, your providers may arrange to mail it to you. But, they do not get to just show it to you, and then keep it themselves. HIPAA requires the Notice to explain to you, in plain language, what your rights are, what your providers responsibilities are to protect your privacy, and it must tell you how your healthcare information can be used and shared. If the Notice doesnt tell you your rights, insist on getting one that does. Next, you have the right to look at and get a copy of your entire health record that is maintained by each of your providers. Why would you want a copy of your health records? Because unscrupulous employees at doctors offices can — and do — steal health information and use it to steal identities, or sell it to others who will. Example: in 2004, an employee of a cancer center in Seattle stole the identity of a patient, and used the information to spend over $9000 on fake charge cards. That person was sentenced to 16 months in federal prison. Example: just last year, a disgruntled former employee in Providence stole the health information of 200,000 patients. He was caught before he could do any damage, and was recently indicted. There are many more examples of stolen health information. What if your information is stolen from your provider? If this should happen, your provider must notify you so that you can take the necessary steps to protect yourself, such as run a credit report, check for fraudulent use of your credit cards and bank accounts, etc. So, you should request a copy of your health records from all your providers. They are required to give you the copy within 30 days of your request. If your providers cannot give the copy to you within 30 days, they must notify you of their reasons in writing, and then give you a copy within another 30 days. If your providers do not give you a copy of your health records within a total of 60 days, you can complain to them, or you can file a complaint directly with Health & Human Services. You may ask for and receive as many copies of your health records as you like, as often as you like. Your rights do not limit the number of copies you may have. Your providers are allowed to charge you a reasonable, cost-based fee for the copying and postage. Some providers will give you the first copy of your records at no charge, but charge you for additional copies. Other providers will charge you for every copy. What if you get a copy of your health record and notice that something is not correct? You now have the right to ask your healthcare providers to change the information in your health record if you think that it is incomplete or is not accurate. They can refuse your request to change the information, but if they do, they must furnish the reason for the denial to you in writing. If you are given a written denial, then you have the right to submit your reasons for disagreeing with the denial, and your reasons must be added to your health record. Your healthcare providers are allowed to share your health information with other providers. For example, when your primary physician refers you to a specialist, only the appropriate parts of your health record are given to the specialist so that you can be given the best treatment possible. In the same way, your providers are allowed to share your health information with your insurance companies, so they can be reimbursed for your care. Your providers are not required to ask for your authorization in order to share your health information with your other providers or your insurance companies, though it is very common for them to do so. However, there are a number of other situations in which your provider is legally allowed to share your health information — without your authorization. Do you know what they are? For example, all states require your providers to disclose information about certain communicable diseases to your local health department. Your providers may disclose this information without your authorization, but they must also track, or otherwise account for, the disclosure. Other examples include disclosures to law enforcement officials when requested with a court order, court-issued warrant, or subpoena. Your providers may disclose the requested information without your authorization, but again, they must also track or otherwise account to you for the disclosure. There are several other kinds of disclosures your providers must track, and the Notice of Privacy Practices that your providers give to you should tell you specifically what they are. If the Notice doesnt give you this information — then ask that it be provided to you. The most important thing you need to know about the disclosures that can legally be made without your authorization is that your providers must track those disclosures, and they must tell you about them whenever you ask. Remember, your providers do not have to keep track when they share your health information with other providers who are also caring for you, and they do not have to keep track when they share your health information with your insurance company for reimbursement, or payment, reasons. But any other kind of disclosure that is made without your authorization must be tracked, and you have the right to know what those disclosures have been. And dont forget — if your health information is stolen or otherwise disclosed illegally, your providers must notify you. Next, you have the right to restrict with whom your providers may share your health information. But, your providers are not necessarily required to agree to your restrictions. For instance, you can ask that your providers not share your information with certain members of your family. Most providers will agree to this kind of restriction. However, they will not usually agree to a request to restrict disclosures they need to make to other providers for your treatment, nor will they agree to restrict disclosures to your insurance company. And, your providers always have the right to share your health information in case of an emergency. Last, you have the right to ask your healthcare providers to communicate with you through a particular address or phone number. For example, if you dont want them to call you at home with test results, you may ask them to call you at work instead. Or, you may not want to receive test results on a postcard, but instead want them mailed to you in a sealed envelope. As long as your request is reasonable, your providers must communicate with you in the way that you ask. Congress and Health & Human Services have given you some significant rights with respect to your healthcare information. These rights are important tools that you can use to help protect your health and personal information. No one is a better watchdog for your information than you. I simply mean that you should eat smaller portions, and your meals should be balanced, consisting of healthy foods from every food group.) You may try only cutting down on all the high-calorie foods and skip the exercise. You may cut down on your portion sizes. And, the weight will eventually come off. However, if you also include exercise in your weight loss program, youll discover that not only will the weight come off, but your metabolism will improve, your fat will be replaced by lean, muscle tissue (rather than loose, jiggly flab), youll feel more energetic and mentally alert, and youll be better able to keep the fat off. The human body was created to be active. It wasnt meant to live a sedentary lifestyle. Unless your health prohibits you from getting some sort of exercise each day, you should make an attempt to focus on the amount of physical activity you engage in. After all, most Americans dont get nearly enough physical exercise during the course of a day! Were all too busy sitting at our desks or in front of our televisions. By the same token, if youre like the average American, who thrives on fast food or other high-fat, high-calorie meals, a change in eating habits is advised. You can still enjoy eating! Instead of that 44 ounce Coke, drink more water. Try to include more fish in your diet, rather than a big, fat hamburger. Youll be surprised at how much better you look and feel, when you replace those heart-clogging, fat-producing calories with healthier food choices. And, dont skip meals! If you regularly skip meals, your metabolism is going to slow down, youll feel sluggish, and your body is going to hang onto those extra pounds as protection against starvation. By eating regularly, your metabolism works better, and youre better able to lose weight! Yes, diet and exercise are important ingredients for successful weight loss! You dont have to be miserable in the process of losing weight. Just remember the following points, if you want to shed those pounds and keep them off: Dont starve yourself! Eat regularly to ward off hunger. Focus on eating smaller portions (perhaps several small meals each day would work best for you). Make sure that the calories you consume are nutritious and include foods from each food group.Find an activity or two that you enjoy, so that exercise time isnt dreaded. If you enjoy what youre doing, youll be more likely to stick with your exercise routine.
